Claim: The ornamental design for a combined bottle and cap, as shown and described.
Description: FIG. 1 is a side elevational view of a combined bottle and cap showing our new design, the opposite side being a mirror image thereof;

FIG. 2 is an elevational view looking from the left of FIG. 1;

FIG. 3 is an elevational view looking from the right of FIG. 1;

FIG. 4 is a top plan view; and

FIG. 5 is a bottom plan view thereof.
